To officially schedule your child's Baptism, the parish requires the following items to be completed and submitted:

Baptism Registration Form: Completely filled out and signed.
Child’s Birth Certificate: A copy must accompany your registration form.
Preparation Class (Parents): Attendance is mandatory unless a class was taken in the last two years. If so please provide documentation of course certification.
Preparation Class (Godparents): Attendance is mandatory. If they are not local, they can take a class at their home parish or complete an approved online Baptism course. We can provide a link if necessary.
Godparent Eligibility: You may choose one or two godparents (if two, one must be male and one female). Catholic godparents must be at least 16 years old , fully Confirmed, practicing Catholics, and provide a Certificate of Eligibility from their home parish.
Christian Witness: A baptized non-Catholic Christian may serve alongside a Catholic godparent as a Christian Witness.
Parish Permission: If you are not registered members of Our Lady of the Rosary, you will need a permission letter from your current parish pastor.
Special Documentation: Legal adoption or guardianship paperwork is required if applicable.
